YUNGLING DISEASE

From Big Medical Encyclopedia

YUNGLING the DISEASE (O. Jiing-ling, is mute. surgeon, 1884 — 1944; synonym: a multiple cystic tubercular osteitis, a multiple cavernous tubercular osteitis, ostitis tuberculosa multiplex cystica) — one of rare forms of tuberculosis of bones which is characterized by existence of the multiple destructive centers, hl. obr. in phalanxes of fingers of brushes and feet.

Jung-lingom is for the first time described in 1908 by N. V. Morozov and in 1919. About the tubercular nature Yu. testify allocation of mycobacteria of tuberculosis from the centers of defeat, existence of accurately expressed transitional forms between typical displays of tuberculosis of bones and a disease of Yunglinga. However in literature there are statements about not tubercular etiology Yu.; assume, in particular, that this pathology represents chronic inf. reticuloendotheliosis of not clear etiology. At gistol. research patol. the centers find their looking alike a tubercular granuloma, but at the same time there is no caseous necrosis and large sequesters do not come to light.

The disease is more often observed during puberty, however its beginning is possible at children's age. Long time Yu. proceeds asymptomatically and it is quite often diagnosed only at emergence of deformations of bones or patol. changes. The most frequent localization of defeat — a phalanx of fingers of top and bottom extremities; occasionally pelvic bones, vertebrae and bones of a skull can be surprised. At rentgenol. a research reveal the roundish centers of an enlightenment of various size, generally in metaphyses of bones (fig). Sometimes all is surprised or almost all bone of a phalanx of a finger. Lack of periosteal reaction is characteristic. Distribution patol. process leads to the expressed deformations of phalanxes of fingers, their shortening and patol. to changes.

BB. differentiate with a chondromatosis of bones (see), at Krom find symmetric damages of the bones of the same name and a yacheistost of structure of affected areas more often.

of an enlightenment in the field of metaphyses of metacarpal bones and phalanxes of fingers are visible.

Treatment Yu. essentially does not differ from treatment of other forms of tuberculosis of bones. Preferably conservative treatment — antibacterial therapy by antituberculous remedies (see). During the progressing of process patol. the centers delete in the operational way.

The forecast is more often favorable. In most cases there comes treatment. At early recognition of a disease and an early treatment it is possible to prevent development of deformations.
