SPIRADENOMA EKKRINNY (Greek speira a ball + aden of iron + - oma; to allocate Greek ekkrino; synonym knotty adenoma of sweat glands) — rather rare benign tumor of skin, a gistogeneticheska connected with ekkrinny sweat glands. As the independent nosological form is allocated in 1956 by D. W. Kersting and Helvig (E. V. to Helwig). In literature it is described apprx. 300 observations of S. e.

Page e. occurs approximately equally often at men and women, 40 years preferential are aged more senior though arises also at children's and senile age. It is localized on any site of skin, excepting palms, a nail bed, an areola of nipples of mammary glands, vulvar lips and a prepuce; damage of skin of hands, the head and an upper half of a trunk prevails. Approximately in 10% of observations of S. e. happens multiple.

S. e is macroscopic. represents knotty education, is located in a derma and occasionally in hypodermic cellulose, it is covered with not changed, cyanotic or reddish skin. Page e. it is, as a rule, accurately delimited from surrounding fabrics, has the oval or rounded form, the small sizes; occasionally to 6 cm in the diameter. A node on a section of pinkish color, sometimes with reddish sites or cysts.

S. e is microscopic. consists generally of cellular elements of two types: rather small «light» cells (with light puzyrkovidny kernels and moderately basphilic cytoplasm) and smaller «dark» cells (with hyperchromic kernels and scanty, almost indiscernible cytoplasm). In a quantitative sense «light» cells prevail, and a glycogen, mucin and lipids in them do not come to light. The stroma of a tumor, as a rule, scanty is also presented by reticulin and collagenic fibers. Sometimes vessels of sinusoidny type meet peculiar expanded circulatory or limf. Focal lymphoid infiltration is often observed.

Relative positioning of cellular elements C. e., their group, and also nek-ry features of a stroma substantially define its structure and allow to distinguish different options of a structure. Sites of a solid structure, educated or randomly located cells of both types, or alveolar accumulations of «light» cells meet with the correct ranks of small «dark» cells on the periphery more often. Also formation of tubular, ferruterous, cystous, sosochkopodobny and klubochkopodobny structures is noted. In all these structures the characteristic arrangement of cells comes to light: in the central departments — «light», in peripheral — «dark». Depending on dominance of these or those structures allocate solid, tubular, the adenomatous (ferruterous), angiomatous and mixed options C. e. At gistol. research C. e. it is necessary to differentiate with bazaliomy (see) and tsilindromy (see) skin, akrospiromy, a glomusangioma (see. Glomal tumors ) and other vascular tumors.

Very often patients note morbidity at hiting at of a tumor or cooling, but quite often pains have also spontaneous pristupoobrazny character. Occasionally, in the main ambassador of an injury, S. e. can ulcerate. Wedge, current long, growth of a tumor slow.

Treatment operational.

Forecast favorable. A recurrence is observed seldom, preferential at an incomplete oncotomy. Repeated reditsiva are noted.
