PUERILISM

PUERILISM (Latin puer the child) — a psychopathological state with regress of mental activity (emergence in the speech and behavior of the adult of the lines inherent to children).

The term «puerilism» offered Dyupre (E. Dupré, 1903). At P. patients speak with children's intonations, lisp, lisp, childly build phrases, address all on first-name terms, call «uncles», «aunts». They express joy at the sight of bright brilliant objects, reach for them hands, willingly play childish sports (build lodges of paper, collect pictures, play dolls). Often are capricious, discontentedly pout, with insult cry. Quite often on this background separate elements of a pseudodementia are noted (see. Pseudo-dementive syndrome ). So, patients cannot cope with elementary tasks, the elementary arithmetic operations carry out with appreciable errors. However in general behavior of the patient never to identically usual children's behavior. It differs in characteristic is dissociated-nostyo: along with children's lines in behavior of the patient separate habits and skills of the adult, napr, the correct manner to light matches, to light remain.

Most often P. is one of forms or stages of development of hysterical reactive psychosis (see. Reactive psychoses ). At this P. arises after a psychogenic depression or a pseudodementia and can be replaced in process of further deepening of a reactive state by a syndrome of «running wild», at Krom regress of mental activity reaches extreme degree (patients grabble, eat with hands from a plate, growl, bark, bite passing for legs, etc.). In some cases P. is replaced by a psychogenic stupor (see. Reactive psychoses). Sometimes the reactive state throughout all disease keeps a complete same form P. Quite often P. meets at schizophrenia (see), especially at its emergence at youthful age and existence in a wedge, a picture of hebephrenic frustration. The item is noted at organic diseases of c. N of page as accompanied with dementia (senile dementia, a general paralysis), and without the expressed its signs (at tumors of a brain, especially at frontal localization). Tranzitorny P. can be observed in the acute period of a craniocereberal injury, at concussions and especially at the injury caused by a blast wave.

Treatment it is directed to a basic disease.

Forecast The item at schizophrenia and organic diseases of c. the N of page is defined by a current of a basic disease. The forecast of P. which arose after a mental injury (psychogenic P.) is always favorable though the disease can accept a long current.

Forensic-psychiatric assessment at a psychogenic puerilism in the period of a disease it is not taken out — the patient is sent to mental health facility for treatment; at the puerilism which arose against the background of other mental diseases, sudebnopsikhiatrichesky assessment is defined by character of a basic disease.
