OFTALMOPLETIZMOGRAFIYA (grech, ophthalmos of eyes + a pletizmografiya) — a method of record and measurement of the fluctuations of volume of an eyeglobe arising during a cardial cycle. It is for the first time offered by Til (R. Thiel, 1928).
Change of bulk volume of blood in intraocular vessels in various phases of a cardial cycle is shown in the form of fluctuations of volume of a fibrous (corneoscleral) cover of the eyeglobe (volume eye pulse) registered by means of the device — an oftalmopletizmograf.
The lake is used for assessment of level of blood supply of eyes and a condition of walls of intraocular vessels at glaucoma, an angiosclerosis of a retina and an idiovascular cover, the general atherosclerosis, an idiopathic hypertensia, and also for diagnosis of occlusal processes in system of carotid arteries. O. at inflammatory processes in front and back departments of an eye, dystrophic changes of a cornea, decompensation of intraocular pressure, dislocation of a crystalline lens, hemorrhage on an eyeground, amotio of a retina is contraindicated.
Islands carry out in the lying provision of studied (fig. 1) under local anesthesia of an eyeglobe (0,5% solution of Dicainum). On the head of the patient strengthen nalobny adaptation with a stripper of a pulsation in the form of the hollow cylinder, to-ry establish about a limb and use also for a compression of an eyeglobe. Registration of eye pulse is carried out at gradual increase in the intraocular pressure reached by a compression of an eyeglobe. Determine the change of volume of all eyeglobe which is approximately corresponding to the size of pulse volume of blood in vessels of an eye by the calibration device and the subsequent simple calculations. Approximately the size of pulse volume of eyes of healthy people of middle age varies from 11 to 17 mm3. The size of pulse volume less than 10 mm3 indicates deterioration in blood supply of eyes. The relation of size of amplitude of the minimum pulse wave to amplitude of the greatest wave on an oftalmopletiz-mogramma is normal not less than 1:2. At decrease in elasticity of walls of intraocular vessels this relation increases. The expressed flattening of tops of pulse waves is considered as a sign of a sclerosis of intraocular vessels.
During the use of permissible compression load and the correct carrying out O. of complications it is not noted.
Oftalmopletizmograf — the device intended for quantitative assessment of pulse volume of blood in intraocular vessels. The principle of its action is based on measurement and registration of a pulsation of an eyeglobe under the influence of the loading enclosed on it.
In the USSR the oftalmopletizmograf developed in All-Union scientific research institute of medical instrument making and the Moscow scientific research institute of eye diseases of Helmholtz (fig. 2) is applied.
The manual and automatic compression of an eyeglobe is provided in the device. In the first mode the set size of loading is created by turn of the handle and controlled on a scale of counting. In the second mode by means of the lever gear drive set in motion by the electric motor uniform increase of load of an eyeglobe ranging from 5 to 100 gs is provided (-0,05 H to — 1,0 N).
Quantitative measurements of elements of an oftalmopletizmogramma are carried out by comparison of amplitude of pulse waves with the dosed changes of volume with the calibrator. Growth of amplitude And pulse waves (fig. 3) happens at increase of loading approximately that testifies to normal reactivity of intraocular vessels. Decrease in amplitude of pulse waves, flattening of tops and insufficient increase of amplitude at loading are observed at various diseases which are followed by disturbance of blood supply of eyes and change of a condition of walls of intraocular vessels (glaucoma, a hypertension, atherosclerosis, etc.).
