MAYOKKI DISEASE (D. Majocchi, ital. dermatologist, 1849 — 1929; synonym ring-shaped telangiectatic purpura) — the rare dermatosis relating to hemosideroses of skin.

Mayokki in 1895 is described.

Etiology it is not clear; attach significance to cardiovascular pathology (hypertensia), to infectious and toxic factors (hron, tonsillitis, tuberculosis), to endocrine disturbances (diabetes mellitus), to disturbance of exchange (gout), to injuries (pressure, friction by clothes).

Epidermis is a little thinned. In a papillary layer of a derma emergence of a significant amount of neogenic capillaries, expansion of their gleam, puffiness of walls, proliferation of an endothelium, emigration of erythrocytes, adjournment of hemosiderin in perivascular fabric and histiocytes is observed. Fibrinoid changes of connecting fabric are absent. At a kapillyaroskopiya aneurysmal expansion of capillary loops and delay of a blood-groove in them is found.

The clinical picture

Is observed preferential at men of young age. In development of a disease allocate three stages.

1. The stage of teleangiectasias is characterized by emergence around hair follicles of several grouped specks size from a point to 0,5 cm in the diameter, brightly red color caused by a trichangiectasia, formation of teleangiectasias and the smallest hemorrhages (during the pressing of a spot completely do not turn pale).

2. Hemorrhagic - the pigmental stage differs in formation of considerable number of dot hemorrhages. The centers of defeat increase due to merge of these hemorrhages; at the same time they are resolved in the center, getting a characteristic form of the not closed ring having a diameter of several centimeters.

3. The stage of an atrophy is most expressed in the center of spots where skin gets pale yellow coloring, becomes brilliant, thinned, openings of hair follicles smooth out.

One or several centers of defeat are localized preferential on skin of shins, in anklebones, more rare on buttocks, a waist; sometimes process accepts widespread character. Subjective feelings in the centers of defeat are not noted. M.'s current. usually long, from several months to several years, with a tendency to gradual permission of skin manifestations; perhaps subacute development of a dermatosis.

Diagnosis it is put on the basis of data of a clinical picture of a disease.

Differential diagnosis it is necessary to carry out with other forms of a hemosiderosis of skin: with Shamberg's disease (see. Shamberg disease ). at a cut the ring-shaped centers are not formed; with the arc-shaped telangiectatic purpura differing in development of the closed center of defeat; with a hemosiderosis at a varicosity (see. Hemosiderosis of skin ); with medicinal rashes (see. Toksikodermiya ).

Treatment and Forecast

Treatment. Treatment of a basic disease is carried out. Drugs of calcium, redoxons, River are shown. At limited forms of a disease repeated applications or massage by snow coal to - you, irrigation by Aether chloratus are recommended, to ointment with corticosteroids.

Forecast favorable.
