GIGRONY (Hygronium; joint venture. B) — ganglioblokiruyushchy means of short-term action. Dimetil-amino ethyl ether N - methyl - alpha pyrrolidine carboxylic acid dimethoiodide:

White or inodorous slightly cream fine-crystalline powder. It is easily dissolved in water, it is not enough — in alcohol and it is almost insoluble on air. On light turns yellow. Aqueous solutions are unstable.

The basic pharmakol, property G. is its ability to block N-holinoreaktivnye of structure vegetative gangliyev. Surpasses in ganglioblokiruyushchy activity of G. a little petrolhexonium (see), is less toxic.

Blocks both sympathetic, and parasympathetic ganglions. Thanks to oppression sympathetic gangliyev G. causes a lowering of arterial pressure. At the same time under the influence of G. pressor effect of adrenaline amplifies a little. Unlike other drugs of this group, G. has no atropinopodobny effect, does not cause myotropic spasmolytic effect. Drug does not increase the content in blood of a histamine, has no antihistaminic and kurarepodobny effect. More than arfonad, stimulates processes of oxidizing phosphorylation and, unlike it, lowers consumption by fabrics of oxygen that probably creates conditions for more economical expenditure of energy resources. In the conditions of the ganglionic blockade and hypotension caused by G., the phenomena of a fabric hypoxia and acidosis do not develop.

Use for the managed hypotonia during surgeries on heart and the main vessels, concerning coarctation of an aorta, an open arterial channel, a renovascular hypertension, and also during removal of vascular tumors; in neurosurgery at various brain operations; at a fluid lungs (in complex therapy); in obstetric practice at the complications of pregnancy which are followed by high arterial hypertension (a nephropathy, a preeclampsia, an eclampsia) and also at diseases of the pregnant women and women in labor who are followed by substantial increase of arterial pressure (an idiopathic hypertensia, coarctations of an aorta, etc.).

The drug is administered intravenously kapelno in the form of 0,1% of solution on isotonic (0,9%) solution of sodium chloride with a speed of 70 — 100 thaws a minute in the beginning, on reaching the necessary level of hypotension rate of administering is reduced to 30 — 40 drops a minute. Hypotensive action develops in 2 — 3 min. After the termination of administration of drug initial arterial pressure is recovered in 10 — 15 min. At single or fractional introduction (on 40 — 80 mg of drug in 3 — 5 ml of isotonic solution of sodium chloride) duration of hypotension after each injection — 15 — 20 min. At an excessive lowering of arterial pressure it is possible to use adrenomimetichesky means (adrenaline, noradrenaline, a phenylephine hydrochloride, etc.).

At G.'s use negative side effects usually are not observed. Tachycardia, temporary insignificant reduction of number of erythrocytes and leukocytes and increase in number of reticulocytes is sometimes noted; can raise several coagulability of blood.

Forms of release: the bottles or ampoules with a capacity of 10 ml containing 0,1 g of nonvolatile solid. Dissolve drug just before use. Keep in the place protected from light.
