GESSING DEVICE (F. v. Hessing, is mute. the mechanic-orthopedist, 1838 — 1918) — pivotally-gilzovy an orthosis for prevention of deformations, and also unloading and fixing of the lower extremity. Gessingom in 1870 is offered.
Depending on a functional condition of the lower extremities Gessing developed several versions of the device. Their distinctiveness is the possibility of more careful individual adjustment of already ready device by shift of tires and sleeves. The device is used at sluggish and spastic paralyzes of various etiology, static and inborn deformations of an extremity (hl. obr. after the postponed rickets or an arthrogryposis), at dystrophic and infectious diseases of joints, gemofilichesky arthritises, the dangling coxofemoral or knee joint etc.
the Device is produced from skin, a soft lining and metal tires on a plaster cast of the affected extremity. It consists of a pelvic part, sleeves of a hip and a shin, and also a shoe (fig). Pelvic part in the form of a semi-corset fastens to a femoral part of the device by means of belts, the coxofemoral hinge and a trochanter providing the correct axis of movements of an extremity and limiting laterpositions of a trunk during the walking. The semi-corset is covered from the inside by a flannel and thin skin; in an inguinal part a laying more dense. Sleeves of a hip and a shin on an outer and inner surface are supplied with metal tires which at the level of coxofemoral, knee and talocrural joints are connected by hinges thanks to what in these joints partial or full mobility is provided. If necessary the movement in hinges can be switched off by means of locks or other special devices. On a back surface of a sleeve are supported with additional thin crosswise tires with special plates on the ends and openings for screws, to-rymi they are screwed directly on sleeves of the device or on side tires. Tires of a shin do sliding.
On a sleeve of a hip from above there is a device for an emphasis of a sciatic hillock that provides unloading of an extremity during the standing and walking. For fixing of the device on extremities of a sleeve of a hip, a shin and a shoe on a front surface have a lacing, edges are broken less by blood circulation of the affected extremity, than tightening belts. At sluggish paralysis of muscles of the lower extremity the otvisaniye of foot is warned by the rubber tyazha going from a front surface of a shoe on a sleeve of a shin.
G.'s advantage and. consists in good fixing of a basin and a possibility of use of the device even at the expressed deformations of coxofemoral and knee joints; existence of side and crosswise tires allows easy shift of both sleeves forward and back, up and down that facilitates individual adjustment of the device and allows to use its unlimited time (length of the device can be increased in process of growth of the patient). Lack of the device is complexity of production and its considerable weight. For this reason of G. and. in a typical look it is applied seldom. The greatest distribution was gained by the facilitated, so-called bezzamkovy devices, the developed N. A. Schenk, I. M. Mitbreytom, I. K. Shepelev and other researchers, and also the devices with the lock in knee hinges assembled according to the scheme TsNIIPP (Central nauch. - issled, in-t of prosthetics and prosthetic engineering).
