EZOFAGOTONOGRAFIYA

From Big Medical Encyclopedia

EZOFAGOTONOGRAFYYa (Greek oiso-phagos ppshchevod +tonos tension + grapho to write, represent; synonym:

an ezofagomanometriya, an ezofagomanogra-fiya) — a method of graphic registration of pressure in a gullet. It is applied to diagnosis of diseases of a gullet, at to-rykh pressure in a gullet and a tone of sphincters of a gullet (changes at an incompetence of cardia, the sliding hernias of an esophageal opening of a diaphragm, etc.).

For record of an ezofagotonogramma use thin (usually two - or three-channel) the probe, the distal end to-rogo (free or terminating in a thin-walled rubber bulb) enter into a gullet, and proximal — connect to the data-acquisition equipment (Marey's capsules, float-operated and manometrical system, etc.) - The sensors registering pressure establish in certain sites of a gullet or in a zone of its sphincters. Then ask the patient a nek-swarm time not to do deglutitory movements and make record of pressure in a gullet. Ezofagotonogramma is registered at gradual slow extraction of the probe and reflects a so-called profile of pressure in a gullet. Along with the waves caused by changes of a gullet the waves connected with the deglutitory movements, deep breath and a cordial pulsation can be registered. Along with the help of special cuffs, to-rye put on a breast and a neck, register the respiratory and deglutitory movements for the purpose of differentiation of the waves arising at spontaneous reductions of a gullet, and the waves connected with the deglutitory movements and deep breath of inspected. Pressure in a gullet slabootritsatelny with fluctuations is normal during a breath and an exhalation within 6 mm of mercury. Pressure is increased in the field of a pharyngoesophageal sphincter (an upper zone of supertension) and an esophageal and gastric sphincter (the lower zone of the raised

Fig. of Ezofagotonogramm of area of an esophageal and gastric sphincter is normal (and), at the sliding hernia of an esophageal opening of a diaphragm (b), at an incompetence of cardia (in): 1 — the lower zone of supertension (normal it is defined at the level of a diaphragm, at the sliding hernia of an esophageal opening of a diaphragm it is displaced in the proximal direction — • in the drawing to the right, at an incompetence of cardia is absent); 2 — waves of reduction of a gullet during the swallowing; 3 — waves of a cordial pulsation; 4 — the waves arising at breath; the dashed line designated the level of a diaphragm.

pressure). In a wedge, practice the greatest value has determination of pressure in the field of a pishchevodnozheludochny sphincter which is created by a tonic muscle tension of a lower part of a gullet (fig., a). At the sliding hernia of an esophageal opening of a diaphragm on an ezofagotonogramma the shift of the lower zone of supertension is observed up (fig., b). At an incompetence of cardia as a result of damage of cardial area at an operative measure or patol. process the lower zone of supertension disappears (fig., c).

AA. it is contraindicated at тяя^елых diseases of cardiovascular system, a respiratory organs, a nasopharynx and the gullet which are followed by difficulty of swallowing, pregnancy, acute inflammatory diseases of abdominal organs.
