HIMOPSYN (Chymopsinum) — the fermental drug containing mix of A-chymotrypsin and trypsin. Receive from a pancreas of the lethal cattle.

Powder of color, white or white with a yellowish shade. Let's easily dissolve in water and isotonic solution of sodium of chloride; 0,2% water solution have pH 4,5 — 6,5. In water solutions chemical opsin is unstable, decays at high temperature, at t ° 2 — 5 ° activity of drug in solutions remains within a day. Colourless transparent or slightly opalescent solutions of chemical opsin without deposit are allowed to use.

Due to the existence in composition of chemical opsin of trypsin (see) and A-chymotrypsin (see Chymotrypsin) drug renders same pharmakol. action as the specified enzymes. It promotes dissolution of clots, deposits of fibrin, a не-кротизированных of fabrics, and also causes fluidifying wound separated and viscous secrets (e.g., phlegms), reduces resistance of pyogenic bacteria to action of antibiotics. During the drawing on purulent wounds accelerates process of regeneration. Practically does not affect the unimpaired fabrics since they contain specific and nonspecific inhibitors of trypsin and A-chymotrypsin. Unlike drugs of these enzymes chemical opsin is less purified and therefore it is allowed only for topical administration (on wound surfaces, etc.) and for inhalations.

In surgical practice chemical opsin is applied to treatment of purulent wounds, trophic ulcers, burns and decubituses. For this purpose 0,025 — 0,05 g of drug dissolve 0,25% of solution of novocaine in 10 — 50 ml and moisten with the received solution sterile napkins, to-rye impose on a surface of a wound. By the same technique it is possible to apply 0,5 — 1% solutions of chemical opsin in isotonic solution of sodium of chloride. In the presence of liquid separated in a wound chemical opsin sometimes enter into it in a powdery look (up to 0,2 g). At decubituses and trophic ulcers drug is used in the same way as at treatment of purulent wounds. In the presence of thick strup the last cut.

In Ophthalmolum. to practice himepsin apply to treatment of a keratitis and helcomas. For this purpose drug is appointed in the form of 0,2% of solution for trays or in the form of 0,25% of the solution used as eye drops, to-rye appoint 3 — 4 times a day within 1 — 3 days.

As the expectorant of direct type of action chemical opsin is applied at various diseases of lungs (pneumonia, bronchitis, etc.). Drug is appointed usually in the form of inhalations of 1 — 2 time a day. For inhalations of 0,025 — 0,03 g of drug dissolve in 5 ml of sterile isotonic solution of sodium of chloride or a distilled water. Sometimes such solutions of chemical opsin enter intratrakhealno via the bronchoscope or the endobronchial probe. After inhalation or intratracheal introduction it is necessary to track that the patient in the next few hours after the procedure carefully expectorated a phlegm. At inhalation use to solutions of chemical opsin it is possible to add broncholitic means and antibiotics.

Side effect of chemical opsin is shown by hl. obr. passing subfebrile condition and allergic reactions in this connection before its use sick it is reasonable to appoint Dimedrol (see), Diazolinum (see) or any other antigistamipny substances (see). After inhalations sometimes there is a hoarseness of a voice, edges passes independently. At use of chemical opsin in the form of eye drops and trays there can be an irritation and puffiness of tissues of eye that demands reduction of concentration of drug, and in hard cases — its cancellations.

Contraindications to use of chemical opsin same, as to use of trypsin and chymotrypsin (a decompensation of cordial activity, dekompensirovanny forms of a pulmonary tuberculosis, infectious hepatitis, cirrhosis, etc.).

It is impossible to appoint chemical opsin for parenteral administration. Locally it is not necessary to apply drug on ulcerated surfaces of malignant tumors.
